The Nook Nutritional Assessment (NAQ) At The Nook, nourishment is more than what’s on your plate — it’s how your body, mind, and spirit receive and restore balance. Our Nutritional Assessment Questionnaire (NAQ) offers a holistic look at your well-being, helping us understand where your body may be asking for deeper care. The NAQ takes a snapshot of where you are in your health and provides an objective reflection of your current health status. Through a series of 300+ reflective questions, we explore patterns in digestion, lifestyle, and more. Your responses help us identify nutritional needs and create a personalized plan to support your natural rhythm of healing. Simply supply us with an email address to send your computer-generated questionnaire to. You will receive your questionnaire link via email within 2 business. Once received, Ai Yuasa- Green will review your results and send you the report with recommendations. Please allow 3-5 business days for your report to be prepared. *The NAQ and any recommendations that follow are not intended to diagnose, treat, or cure any illness. Please consult your healthcare provider before beginning any new supplements or nutrition protocol.
Cancellation Policy
To cancel or reschedule, please contact at least 24 hours in advance. Cancellation fees will be assessed for no shows for the appointment without proper cancelling or rescheduling. Cancelation Fee: 48+ hours in advance: No fee 24-48 hours in advance: 50% of session fee Less than 24 hours or no-show: Full session fee
